CDR is proud to introduce our long-term care plan advocacy services for folks who wish to leave nursing facilities and return to the community.
CDR, and the long-term care plan advocacy services of CDR, aim to remove barriers to the full inclusion of people with disabilities in the life of our community. We offer free long-term care planning consultations, and we assist with planning for Medicaid eligibility, ABLE accounts, and Pooled Trusts.
